NDIS Funding
Care2Communicate is an approved National Disability Insurance Scheme (NDIS) Service Provider. We provide services across Sydney to NDIS participants of all ages. We are able to support our clients with assessment, intervention and reports using funding that falls under 'Improved Daily Living'.
Who is eligible for a NDIS support package?
To receive a NDIS support package, you must:
- Live in an area where the NDIS is available
- Meet the residency requirements (Australian Resident & citizen, Permanent Visa holders, Protected Special Category Visa holders).
- Meet the disability or early intervention requirements
- Be under 65 years of age when the access request is made
- Meet any other conditions relevant to your individual circumstance

Private Health Insurance
Depending on your health fund, you may be eligible to claim speech pathology services. This will depend on your cover, so its best to check with your health fund prior to commencing session, about the rebate that can be claimed under extras cover.
Chronic Disease Management plan (CDM)
We are also registered with Medicare and are able to see clients using the Chronic Disease Management referral scheme who are eligible to receive a rebate for up to 5 (20 Minute) Speech Pathology sessions per calendar year ( $60.35 per rebate). People of Aboriginal or Torres Strait Islander descent are eligible for 10 sessions per calendar year. The CDM plan provides people with a chronic medical condition a rebate for allied health services. To determine if you are eligible for a CDM plan, you will need to discuss this with your GP.
